Rushcliffe has been chosen by NHS England as one of 50 ‘Vanguards’ across the country to lead significant changes in the way local health and social care services are delivered. As part of our ‘Vanguard’ status we have been awarded national funding to help us improve the quality of local health and social care.
Principia – our local partnership of GPs, patients and community services – is leading this work. Principia has long had a vision to develop a better way to improve the health and care of Rushcliffe residents and now has a wonderful opportunity to turn this vision into reality.
For NHS services to remain free and fit for the future, they need to change and work closely with other services such as social care.
Our ambition is to establish a Multi-specialty Community Provider (MCP) organisation for Rushcliffe that will change how health and social care services are delivered. We want to use the knowledge and experience of the doctors, nurses and the people that patients and carers see every day to make things more straightforward, consistent and accessible - with health and social care finally starting to work together in the way you would hope and expect them to.
Services coming under one umbrella organisation will facilitate more integrated working between these services and promote a culture of ‘mutual accountability’ for improving patient experience and outcomes. A new contract for these services will make it everyone’s business and responsibility to deliver joined-up care that is focused on people’s individual needs and them having a positive experience of care.
What will be different?
Local health and social care services will be fully integrated and provide more joined-up, personalised care
There will be a new focus on prevention and self-care – this includes providing information, advice and early support to promote healthy lifestyle choices and help people better manage their own health and wellbeing
Better management of long-term conditions will include patient education programmes to help people manage and monitor their own diagnosed conditions
Some out-patient clinics and diagnostic services will move out of hospital and into community settings such as GP practices and health centres, so care is closer to home and provides value for money
More patients will use technology for online bookings, prescription ordering, access to health records and test results from home, self-monitoring of conditions and consultations with professionals via email, telephone and video calling
An emphasis on customer care as part of training for staff across health and social care
Better communication between patients and care providers such as GPs, hospitals and social care through record and information sharing
Improved care planning and shared decision-making - giving patients more choice and control over their health and wellbeing.
We are committed to involving patients in these changes so that we end up with the right services in the right place. Advertised events, focus groups and questionnaires are just some of the ways we will be engaging with local people to gather insight, experiences and feedback.

The NHS is facing unprecedented challenges but the Rushcliffe GP practices have decided to work hard together to improve our existing services, develop new locally based services and provide the best care we can to local people.
As Independent Chair for PartnersHealth, we will put the patients care at the centre of all our plans. Helping Rushcliffe GP practices excel in customer service and exceed patients’ expectations.
As a local family Doctor I see lots of patients and I will bring that experience to influence change and improve services in Rushcliffe.
